For young people preparing to receive
the sacraments of Reconciliation and Eucharist or Confirmation

If your child is not baptized and between the ages of 7 – 17,
please contact Joanne Rousculp

Preparation for Reconciliation and First Eucharist is for children in grades 2-4 that are registered for the parish Faith Formation and have participated in the program at St. Michael Parish, or another parish or home school, for at least one year prior to registering for the sacrament.

Confirmation preparation is for youth in grades 9 – 12 that are registered for the parish Faith Formation and have participated in the program at St. Michael Parish, or another parish or home school, for at least one year prior to registering for the sacrament.

A copy of your child’s baptismal certificate will be needed in addition to this form.
