The aim of our parish faith formation is reflected in our parish mission statement: "where people are nourished and formed to be the joyful faces of Christ to others". We do this by enabling adults to share their faith confidently and freely. We also strive to equip parents to fulfill the promise made at baptism to raise their child in the faith.

Faith Formation Registration Form 2022-2023 - This form is for everyone from 3 years old and up, including adults. Check off in-person or at-home at the top. The fees on the form are for in-person. Those choosing at-home formation, we ask $20.00 per child for the materials only. Adults who will not be joining in the breakfast, may elect to purchase the book and supplies for $20.00 (also helps with shipping). The second page allows you to volunteer for a ministry within faith formation and youth ministry. We can always use help in our nursery or with preparation of the Sunday breakfast!
Circle of Grace Safe Environment Participation Agreement - Parents will need to read and sign this agreement for their child in Kindergarten through 12th grade. Training packets will be provided for those who prefer to present this material at home. Please make a note on your form if you will want a packet for home use.
Registration for Reconciliation, Eucharist and Confirmation - If you have a child needing to prepare for one or more of these sacraments, be sure to include this form when you register.

For Pre-School (three-four) Pflaum Faith Formation Program: "Seeds" leaflets and What the Church Believes and Teaches Catechism Handbooks for this age level.
Kindergarten and First (ages five-six) Pflaum Faith Formation Program: “Promise” leaflets and catechism handbooks for this age level.
Second and Third (ages seven-eight) Pflaum Faith Formation Program: "Good News" leaflets and Catechism Handbooks for this age level, plus age appropriate Mass booklets from Liturgy Training Publications.
Fourth and Fifth (ages nine-ten) Pflaum Faith Formation Program: "Venture" leaflets and Catechism Handbooks for this age level, plus age appropriate Mass booklets from LTP.
Middle School (6th-8th grades) Pflaum Faith Formation Program: "Visions" leaflets and Catechism Handbooks for these age levels, plus age appropriate Mass booklets from LTP. We also use St. Mary’s Press “Catholic Connections Handbook for Middle Schoolers” and “BreakThrough! The Bible for Young Catholics”.
High School teens will join with adults for the Lectio: Eucharist study with Dr. Brant Pitre. Other resources for teens are The Catholic Faith Handbook for Youth published by St. Mary's Press and The Catholic Youth Bible Revised New American Bible from St. Mary's Press.
The Adult study beginning Sept. 25 is Lectio: Eucharist with Dr. Brant Pitre. This is a video and discussion course that includes a Study Guide.
